The Story
CHAPTER I

It Started with a Promise Broken

Every revolution begins with a single moment of injustice. Ours began at a pediatric practice in Pasadena, Texas.

The Independence Moment

A gifted pediatrician spent seven years building relationships — with children, with families, with an entire community. She was promised the practice she'd helped build. That promise was broken.

She made a different choice. She chose independence. Roots Health dba Nag Pediatrics opened its doors at 3322 Plainview Drive, Pasadena, Texas — right next door. Not out of spite, but from a first-principles calculation: the relationships were real, the demand was there, and no non-compete could undo a community's trust in a physician they loved.

The Complexity Revealed

She was busy from day one. Forty patients a day, six days a week. Nearly a thousand patients a month. But the credentialing wasn't right. The billing was broken — each visit billed individually, claim by claim, with no system.

The practice was clinically thriving but financially hemorrhaging. So we went back to first principles. We fixed credentialing. We rebuilt billing. We started reading the data. We managed through the data. And the practice came alive.

The Beautiful Core

Underneath all the chaos was something extraordinary: a physician who loved her patients, and patients who loved her back.

Birthday clubs for every child. A+ Clubs where kids who earned top grades received $100 gift cards. Summer programs sending eighth-graders to Harvard. In a pediatric practice, a child stays for 18 years. That's not a clinical encounter. That's a covenant.

The Impossibility Equation

Five thousand patients. Forty to fifty HEDIS quality measures each. That's 200,000 unique data combinations — permutations of care that must be tracked for a Patient-Centered Medical Home.

When a colleague fell ill and asked us to take over her practice: 75% of well-child checkups hadn't been completed. Not because anyone was negligent — because the system was designed to drown physicians in complexity until they surrendered their independence.
